Why does the fdc1004 evm breakout board not work when I separate it and wired it back? A screenshot of the GUI is included.

The EVM shows as connected, so you interfacing to the uC.
Please check your solder joints and confirm they are okay.
Also try selecting Autoread with a non-zero sampling rate on the registers page and check the Enable Continuous Multi-channel Measurements box on the Configuration page.
Hopefully that will get you going.

I notice that the evm keep going online - offline.
After I re-weld I2C interface, then it works. and the sensor plate works too. The i2c cable I used just 5cm, is this too long to occur this?

I am not and I2C expert by any means, but I think 5cm should be okay.
