OPT3101: Please review the schematic

Dear TI experts,

My customer have a product which is in mass production, and they use OPT3101 for this product.

Recently they have a failure of OPT3101, INP(25) pin have no output voltage so they cannot control photodiode.

Could you review the schematic and tell me what is wrong?

Currently failure rate is 2, total amount is 20000.

    I don't see a problem with the schematic. If INP doesn't have a voltage out then yes the Photodiode will not work. I suggest replacing the parallel capacitor C9 and see if that fixes the problem. If C9 is cracked or has been damaged it will be a short to ground.
