DCA1000EVM: DCA1000EVM Failure -- All LEDs Blinking

Hi,

I have an issue with the DCA1000 EVM. It seems that it stays stuck in boot-up. When power is plugged in, the lights shown in the photo continuously flash. Details about the setup:
  1. Power supply is 5V 3A
  2. Both RADAR_FDTI and FPGA_JTAG USB ports are not being detected so we are not able to flash
  3. We measured the voltage from the barrel jack in different power switch settings:
    1. When the power switch is in DC_JACK_5V_IN, the barrel jack voltage oscillates between 0 and 3.4V
    2. When the power switch is in RADAR_5V_IN, the barrel jack voltage gets a steady 5.2V
The lights only flash when the power switch is switched to DC_JACK_5V_IN. This data was collected without connecting the DCA1000EVM board to any other board. We tried unplugging and replugging in the power, making sure that the USB cables work. We are hoping to get advice for what to do in this situation.
