AMC6821: Intermittent Power On Fault

Hi,

I am using the AMC6821 Fan Controller in software control mode. The fan work correctly 99% of the times however sometimes when i power on the board i get the following issues:

1. The Fan controller fully turns on the fan ( 100% duty cycle), IC heats up and does not respond to I2C commands/cannot be found on the bus.

2. The fan controller does not turn on and does not respond to I2C commands/cannot be found on the bus.

A power reset of the board solves the issue and it requires a long power down time of the board to replicate the fault. ( I.E first time in the day it turns on it exhibits the fault however successive power on/offs do not replicate the fault)

All fault pins have correct pull-ups.Any suggestions with the possible problems?
