CCS/TMS320F28027: Debug connection error

I m having a TMS320F28027 device, on my PCB, whose schematics is derived from TIDA01292.

when i m doing the test it is giving the following error:

This error is generated by TI's USCIF driver or utilities.
 
The value is '-183' (0xffffff49).
The title is 'SC_ERR_CTL_CBL_BREAK_FAR'.
 
The explanation is:
The controller has detected a cable break far-from itself.
The user must connect the cable/pod to the target.

But after the run is completed it is giving the following error:

Error connecting to the target:
(Error -1041 @ 0xFFFFFF49)
The debug probe reported an error. Confirm debug probe configuration and connections, reset the debug probe, and retry the operation.

Please help me out in the regard, i replaced the cable, checked clock, dont know where the issue lies
