PMP4408: SMPS FLYBACK

We need to design an SMPS Flyback topology with the following specifications:

Input Voltage: MAX 275VAC

Output 1: 12V 5A

Output 2: 5V 5A

Please recommend a suitable transformer and control IC for this design.
