As it states on the TIDA-00721 page, under the image of the patch, next to the description of the design - 

Fully assembled board (shown above) developed for testing and performance validation only, not available for sale.

The design files are provided for your reference and if desired, to order from a board assembly house of your choice.

The reader is an NFC enabled handset (phone) or tablet (most Android models, perhaps the other OS based one now, too) or can be made from:

if you mean for the reader, from TI, then this soft bundle link which is found via one link I provided earlier will suffice.

this will give you MCU and NFC/RFID platform with which to activate the tag/transponder w/temp sensor, that you will have made from the reference design files. 

i did just notice that the NFC/RFID BoosterPack is out of stock on our store, these can be had from here, they have 97 in stock
