I assume this code worked on a BOOSTXL-CC2650MA or LAUNCHXL-CC2650? There is most likely an issue with the stability of your JTAG connection as issues are only occurring during a debug session. Have you submitted your design to SIMPLELINK-2-4GHZ-DESIGN-REVIEWS for further review?

This board was designed by someone else. He used to program the devices in the same way. I have taken over his job. So I face different issues in programing the board every time.
Does the program run as intended, even if the debugger continues to disconnect? You could try accessing targetConfigs/CC2650F128.ccxml -> Target Configuration -> Texas Instruments XDS110 USB Debug Probe -> The JTAG TCLK Frequency (MHz) and use a slower specified value to try for an improved connection. Are you having any difficulty connecting with Smart RF Studio? You may want to do a quick diff compare between the two .ccxml locations as well.

Yes, even I have used the code to program my board. But this time I am running the same code I get this error.
I don't know about smart RF Studio. I have done the connection test in Target Configuration, it goes successful.
Can you connect your CC2650MODA custom board with XDS110 debugger and start Flash Programmer 2 to see what it detects?
Perhaps SWRA534 would be able to provide some more clues. If you can contact the previous designer then they would be able to provide the most insight.

I used XDS110 to program my CC2650MODA custom board. When I check the connection test in the target configuration it goes successful.
This is simply to confirm that the JTAG connection between the XDS110 and CC2650MODA is stable, through means of different programming software such as Flash Programmer 2 or CCS.
