Is it possible to set the TX power of the CC2540 to less than -23dBm? As far as I can see the only 4 options are for -23, -6, 0 & 4.

In the CC2540, the power is controlled by the stack. There are 4 set levels as mentioned by you. You could use attenuators to further reduce the output power. But remember, using external components to reduce output power will also hurt you on the receive side with reduced sensitivity.
Why do you need to be less than -23dBm?
You might be okay using an attenuator in this case to reduce the output power from the fob. As long as the base unit remains the receiver without any attenuation.
