CC2650: Unable to program my Custom Board based on CC2650

Hello,

I designed a custom board for my application based on CC2650.

I connect it to the debugger of the CC2650 Launchpad and used the SmartRFFlash Programmer to see if I can program it.

When I refresh the software it shows CC2650 in connected devices section but when I try to erase it, I get this error (check below). I also tried the cloud CCS and again I get another error.

I thought it might be because of the reset capacitor and removed it (a friend of mine had this issue and removed the 0.1uf reset capacitor and it solved his problem) but it didn't help.

I cheked all connection and all connection are OK. I also soldered the pain GND pad carefully to the IC to have the ground.

In term of the voltage I get 1.67v on VDDR and 1.26v on DCOUPL.

The Reset pin connected to 3.3V with 10K resistor.

I also attached my Schematic and PCB layout for your reference.
