CC2640R2F: custom board hardware initial board bring-up issue

We designed a custom board using cc2640r2f with ti-launchpad as reference design.
while powering up the board the processor didnt response as well as not  drawing a current of 2mA(circuit contains an LED),when we check the voltage on VDDR it  shows 0-0.1V range and also DCOUPL shows 0v but as per   hardware trouble shooting guide (cc26xx-HW troubleshooting-wikki) VDDR should be around 1.6V and the DCOUPL should be around 1.28.
