CC3200 - not able to flash / execute

Hi Team,

I am using CC3200R in my design. I followed the exact reference design given in its datasheet. But i am facing some times it is not able to flash using UART boot mode. I verified RESET, CLK, SOP pins, Internal regulators outputs. Everything fine. 

Sometimes it is not able to flash

Sometimes it is able to flash but execution is not happening.

I am expecting faster reply to how to proceed debugging further.
