WL1835MOD: Custom board with WL1835MOD will not come up

I have a batch of boards that utilize a custom design loosely based on the Beaglebone Black Wireless.  The WL1835MOD chip on these never comes up (times out during kernel loading).  All power supplies are coming up correctly and the Slow Clock is correct but the UART_DBG_BT never goes high after startup.  That pin is only routed to a test pad and nothing else.  The Integration guide notes: "Operational mode is set by default using internal pull of WiLink (90K typical)".  What can cause the internal pullup not to be enabled on this pin?
