CC3220S: Custom Board Testing , Error connecting to the target: (Error -2131 @ 0x0)

Hello,

I am trying to program custom designed CC3220S board. My Board is designed with on Board Debugger same as development board CC3220S LAUNCHXL. The custom board debugger is working fine when connected to development board CC3220S chip. My custom board CC3220S is not responding and here are the findings:

1. In Uniflash, I am getting an error  "Operation failed: Timeout reading data".

2. In CCS i got an error as  "Error connecting to the target: (Error -2131 @ 0x0)" when tried to debug. When i try to do Test Connection check i am getting an error  " The value is '-233' (0xffffff17).
The title is 'SC_ERR_PATH_BROKEN' ".

3. The Reset pin on CC3220S chip is going low for a while, i think reset is working fine.

4. I am getting 1.2 V and 1.8 V on both VDD_ANA pins.

5. I am not getting pulse on RTC clock .

the design is same as dev board and followed all layout guidelines. Please let me know if i need to do any further tests.
