CC2530: Hardware reset by pulling down RESET_N does not work

A customer has made a custom board with CC2530, which can work fine running the SDK examples, but with a problem resetting by pulling down the RESET_N pin.

When shorting the RESET_N pin to GND, there is quite a big chance the device does not reset. And if pulling Debug_clock pin to ground when RESET_N is low, the device will reset.

Please find the schematic and layout as below, as well as the waveform from the RESET_N pin.

What could cause this issue?

    The GPIO pin 2.2 and the Reset are not related in the device. So pulling GPIO pin 2.2 low should not affect reset.

    Is this happening on multiple boards?

    What is meant by there is a "big chance" the board will not be reset 1/100, 50/100, 99/100 times?

    Is the minimum 1us reset timing being met?

    Also, it appears that the ground pad on the back of the device is not connected to ground. Are there layers that are missing from what is shown above? This is important for correct device operation.
