Attached is the datasheet I’m using, I couldn’t find any information in it regarding what to do with unused input pins. I’m hooking the positive supply to 15V and the negative supply to ground.

Please reference pages 8 and 9 on the tech note that I have attached for more information on ESD diode clamping. Additionally, you will not need to worry about phase inversion.
