I am not sure what causes the error in Altium but some simulators do not recognize the initial state condition (IC=).
I removed all such statements from the netlist - please try to re-run your simulation using the attached netlist.

Problem solved! Turns out that TPA3255 circuit is doing it´s job. The noise is coming from the OPA1644 filter circuit. I will have to focus on this issue.

OPA1644 is a superior device. Almost all quad op amp have the same pinout so many op amps including OPA1644 will drop in with no other changes.
The only noticeable difference between TL084C and OPA1644 usage is input common mode range.

In the meantime have you looked at the OPA1644? It can't match the low power specs of the OPA169x, but is a very good 4 channel audio opamp and comes in the TSSOP-14 package.
