Part Number: THS5661A Tool/software: I want to input two signals from a DAC with a differential output impedance of 100 ohms to a modulator with 50 ohms coax. Can I assume that the two cables are each 50 ohms at this time?

I have verified this in the lab per your setup condition (i.e. mode pin pulled-up with 3.3k), and both modes work as expected.
I would suggest that you check the polarities of all the pins (especially the MSB) to see if some pins are inverted…
