LM358A: Unknown TI IC, possibly a op amp

Hi, 

I am trying to figure out what this TI IC is. There are several of these unknown TI ICs in the BK Precision 9140 triple powers supply. I damaged one of these ICs while using the power supply in a measurement. Google seems to indicate that it could be a LM358A. There is another post on this fourm that indicates it could be a LM2904DGKR ( https://e2e.ti.com/support/amplifiers-group/amplifiers/f/amplifiers-forum/283919/counterfeit-lm358a-package-does-not-match-datasheet-dimensions ). Anyone have an idea what this could be?
