Can you confirm if the need is to drive a 12Vpp signal or a 24Vpp signal?  You may need to consider a higher voltage FDA, as our THS4521 in a 5.5V supply part and cannot swing 24Vpp output.  Our THS4561 would be a potential consideration here.  I am building up a circuit for you; can you confirm that you need a gain setting of 1V/V?  

I assume the output common mode of the fully differential amplifier (FDA) will be set by the MPCIE-AIO16-16F?  The different VOCM (output common mode) voltages would need to be set based on the ADC.

I need to drive a Single Ended Bipolar, +/-12V signal, not 24Vpp signal.

Thank you for confirming your input signal is 12Vpp, with amplitude of 6V.  The THS4521 cannot swing an output voltage high enough to provide 12Vpp output.

You can consider the THS4561 and THS2630 as high-voltage options from our FDA portfolio.  Either of these devices can swing 12Vpp differential at the outputs, which would only require 6Vpp swing on the individual outputs of the FDA.
