Thanks for your inquiry. Yes, we do have current shunt monitors that can handle a 40V common-mode voltage. They include the INA168/9 (current output), INA170 (current output), INA193-8 family, INA270-1, and the INA282. Properly sizing the shunt resistor for a 4A load should not be an issue. When sizing the shunt resistor you will want to keep in mind the gain of the device and the device's power supplies, which limit the output voltage swing. You may also be concerned about accuracy. The first indication of the accuracy of a device is the initial input offset voltage. For the aforementioned devices, the INA270-1 has the largest offset voltage (2.5mV max) while the INA282 has just 70uV. Therefore I recommend starting with the INA282.

I just noticed that the title of your post specified 60V, while the post itself noted 40V. For 60V, you may want to disregard the INA168-9 and INA170 for 60V is their maximum common-mode voltage. The other devices can handle up to 80V common-mode.
