Hi, I created a circuit with the help of Some TI assistance and the resulting setup of the circuit bellow. The problem im noticing with the circuit is once the Device receives 5v it gets very hot and when the 12v is plugged in it makes a squealing sound. Ive checked on the connections everywhere and it seems like the real circuit follows the schematic So im wondering if there is anything off with the schematic or something else

Hi,

There are couple of mistakes in the schematics.

1. REF7050 needs more than Vout+ 400mV as the supply. If you want +/-5V from the DAC, supply REF7050 with min 5.4V

2. Also the op amps in the reference path needs to be supplied with min +/-6.5V for +/-5V. I suggest feed Reference Op amp as the same supply as DAC VCC and VSS.

3. Similarly output op amp too. REF7050 can be powered with VCC as well ( make sure its less than 18V )

if you are using tantalum capacitors, please check the polarity of the same. Looks like C1, C2, C20 needs to be checked.

4. Also I am not seeing bypass caps for any of the op amps in the schematics,

I don't know these mistakes can make the sound you mentioned in the post. Verify the connections.
