DAC8750 issues

We have a small manufactory for measuring properties of oil. A lot of time I succesfully use DAC8750 for generate 4-20mA channel of my electronics. Unfortunately now I often (during two years) meet a trouble OUT port of this converter. It doesn't works and I'm watching 100mA (or sometimes 0) in Iout (rather it shows my amphermeter (Metra hit 30M), and it has a verification certificate, correctly work), in this time power supply don't show the same (it show approximately 0 mA). I decide this problem just replacing to new converter. Can you suggest a way to solve this problem?

Notice: we use varnishing to protect printed circuit boards.

    It does seem like the DAC8750 has been damaged. Do you know how the device might have been operating when the device failed? Without knowing more about the operation, it would be hard to guess what might have caused this issue.
