Sorry for the delay, I have finished the review please see my comments below.
1. It looks like the 1.1V power supply is labeled as 1V2. Can you confirm this is meant to be 1.1V?
2. It appears that you are missing a 0.01uF cap for each pin.
3. For the MODE strap are you trying to use the CSI strapping? Because this is not supported by 964.
4. For the high-speed path it looks like you are using a CMC with a coax connection which I would recommend against. Also the negative side termination doesn't seem quite right. We recommend 49.9Ohm resistors.
