It seems your scope is set on 1V per division. For some reason we are not acknowledging 7-bits default 0001000 address.
1). I am assuming AD0 and AD1 pins are floating?
2). Also host is not driving SDA on 9th clock pulse?
3). Does ONET1151L share I2C interface with another part?

(1) Correct, AD0, and AD1 are not connected.
(2) Correct, not driving SDA during 9th clock pulse.
(3) No. The ONET1151L does not share the I2C interface with another device.
I attached the schematic of the interface circuit.

I am assuming you have the actual reference design. Do you also have access to the GUI that came with this design? This reference design is obsoleted and it is not supported. I am wondering if you have run this GUI and there is communication issue between GUI and reference design.

The pdf I downloaded from the TI website (see attachment, page 9) contains a diagram of the circuit. I put the circuit on a PCB. I have no idea as to which GUI your refer. Does the circuit specified in the pdf cause the chip to malfunction?

There used to be a GUI with this reference design. This GUI used SMBus interface to setup the ONET registers and etc. Looking at the schematic of this reference design, please load R12 and R13 to see if you see a difference? You can try using 5K ohm value Rs.
