1. When controlling de and /re with a single signal, should we use /RE = Vcc value?
2. Is it okay to understand that we need to wait at least 4 μs from DE=High to start bit?

Logic input voltages should be VCC or GND.
When both driver and receiver are disabled, the device goes into a deeper power-down mode that takes more time to wake up from. But when DE and /RE always have the same voltage, then one part always stays enabled. So in your case, 4 µs is the correct value.
