ONET1141L: meaning of the Fault condition

Dear all,

I'm facing a problem with the ONET1141L mounted on a PCB of my own.

It seems that, no matter what, the chip turns into Fault state, asserting the FLT output after any communication through I2C protocol.
I've tried several combinations of the register values but every time I get the FLT asserted.
I know it is quite a problem since it can be due to several things (I2C, currents, signals, ...) but as a first step I would like to understand what is the meaning of the FLT and what are the conditions that bring to the asserting.

Bias current is set to 0 and there is no photodiode connected.

On the PCB there are 3 different ONET and the problem is the same on all of 3. The I2C addresses are correctly set through proper PCB design according to the data sheet. Furthermore, I have 4 copies of this PCB and each one has this problem so we can exclude a broken chip or wrong fabrication.

Actually, I've mounted the device also on another PCB of my own (different design but with similar conditions) and the same register options do not give any problems.

    I would recommend checking this list to see if any of these conditions are being met.

    Another step that is probably worth taking is to compare schematic / layout of your previous design with your current design.  We can also do a schematic review / comparison on our end if you would like.  Feel free to DM me the schematic if you don't want it public.
