TUSB2046B: Getting Error USB HUB failed to reset in windows TUSB2046B

Hi,

1.     we are facing issue with IC TUSB2046B in production. out of 20 boards 6 boards fails due to communication error
2.     I am attaching schematic kindly guide us with changes needed in schematicUSB HUB schematic
3.    in Device  manager under universal serial bus controller ,Generic USB hub is showing  yellow error mark in windows 10 
4.     hub only work with all in one PC which are having Blue usb3.0 ports, not working with black usb ports ie usb 2.0
