In the datasheet of DS160PT801, the retimer application is specified by placing the same in motherboard and riser card.
Can it be placed in a add in card?

PCIe is point to point. Therefor there are not multiple end points. Clock is used by the retimer and it is buffered for the single end point.

If we need 4x4 output connector instead od x16 connector, then don't we need a clock buffer to provide 4 RefClk out to four port?

Yes you would need one 100MHz clock signal. Please refer to the PCIe specifications. This clock is part of the PCIe connector no matter whether you use X4 or others such as X16 lanes.
