Another option could be the SN65LVCP22. It's a cross point LVDS in a 2X2 configuration and can be used as a splitter.

If using the device in a 1:2 Splitter configuration, then use the following:
If using Input 0, then make SEL0 and SEL1 = 0
If using Input 1, then make SEL0 and SEL1 = 1

No, a fanout means you are distributing the input signal to other outputs. The SN65LVCP22 is a buffer since it's a differential to differential device.

For the unused differential inputs [Input 0] I would recommend to tie the "+" input to VCC and the "-" input to GND.

Use high-frequency, ceramic, 0.1-µF and 0.01-µF capacitors in parallel at the power supply pin with the smallest value capacitor closest to the device supply pin.

Thanks Josh, SN65LVCP22 will support 4K and 1080p Video, I am using conversion after lvds spillter, to connect to monitor, DS25CP152TSQ/NOPB is better part compare to SN65LVCP22? Because i little bit varied about transmitted speed.

SN65LVCP22 and the DS25CP152 are very similar devices. Aside from some minor spec changes, the main difference is the data rate of the DS25CP152. If the data rate you need is higher than 1 Gbps then I would recommend the DS25 device.
