Do the boards with this problem always experience it? I.e. a good board is always good and a bad board always bad? If you were to take the PHY from a good board and place it on a bad board, does the problem get resolved?

It is difficult to say. Could you provide the full schematic of the PHY? The schematic snapshot that you sent does not show the Power pin connections (IOVDD33 & AVDD33).
