DP83848I: Short across VCC and GND ==> Damaged DP83848I

Tool/software:

We use the DP83848I. The design is created according to the guidelines. 
The power supply is a stable 3,3V. and  the IC has a RMII interface to an STM processor.
On other side it is connected to an RJ45 connector with transformer. 
In 3% of our first boards, they have the same defect reason. Reason is that the Vcc and Gnd have a short circuit on the DP83848I.
If we remove or replace the DP83848I, the short circuit is no longer present.
Does anyone have the same problem or possible solutions to this?
