MAX3232: DOUT1 breaks

We have a problem with MAX3232. 
The RS232 DOUT1 breaks and the levels at V+ and V- do not look normal.
Unfortunately, the defects only occur at the end customer, so the operating conditions are not known. 
In the circuit, the DIN1 pin is open, can this possibly lead to an error?

Is there a chance to have the defective devices inspected by TI to identify the root cause, why the devices break?

    I wouldn't expect this to directly cause damage to the device. Normally leaving an input pin floating will just cause more current consumption.

    Generally if a part goes through FA, it doesn't directly tell why the device broke. Only that it experienced some kind of electrical over stress. 

    Do you have a schematic I can review? 

    Is it just one device that broke?

    Is it possible the pin was shorted to something?

    Does the damage occur after some use or directly after being hot plugged into something?

  • after a redesign of our board involving the MAX3232 the DIN was (accidentally) left open.

    After the redesign our customer started to see the failure.

    DOUT is not working and you can see that V+ and V- are not at the correct level and not stable. It seems that the charge pumps are not working correctly.

    The failure mostly happens at the end customer, but we have seen some cases directly after assembly of the device.

    We can't reproduce the failure in a lab setting.
