TUSB2077A: USB Hub unreliable startup

I have implemented TUSB2077A Hub and I am having startup issues. When it starts properly, everything is working. But sometimes it will not start no matter how many resets and reboots. Then later it works again.

I have researched all related posts and design guides.

My implementation is in an embedded system where #Reset signal also resets 3V3 which powers the TUSB2077A. The loads are all self powered and I am only using the USB data hub to connect to internal peripheral devices. I have tried many different reset designs including reset delays with RC, various pull ups, changing 3V3 to be permanent powered so reset is independent of power, but it is still unreliable.

Here is the actual circuit where reset uses 15K pullup, 0.1uf to ground, and 10k to system reset.

Any advice would be greatly appreciated
