Should unused input of an ISO7220C be left unconnected or gnded.

never leave unused High-speed CMOS inputs floating. You can connected them to Vcc or Ground. Always connect via a 1k to 4.7k resistor to Ground or Vcc. The same is true for Enable pins. These resistors limit the current in the case of unsuspected noise transients, which otherwise might trigger the internal ESD structure and latch-up the device.
