DS91M040: Thermal resistance for DS90M040TSQ

Part Number: DS91M040

Is the junction-to-case thermal resistance quoted in the DS90M040TSQ datasheet a junction-to-case-bottom value (2.8 C/W)?  Can the component be top-cooled?  If so, do you have a junction-to-case-top value?  Thank you.
