TUSB2046b .. getting " USB device not recognized"

Hi ,

   I am using TUSB2046b chip in my design . i had followed the schematics given by the TI site .  

I am using hub in "Self-Powered Hub, Individual-Port Power Management" mode and no eeprom .

I have made EXTMEM pin  high , EECLK left floating  and EEDATA/GANGED i have made groung as  i am using individual port power mode ..

but when i connect the hub to PC  ( windows xp ) i am getting USB device not recognized .  actually  it is detecting the hub but showing some malfunction in the attached device . 

So kindly  help  me  in soving this  ASAP. ..  i am  attaching the schemtic page also .5516.tusb2046b.PDF

    Hi illiayaz,

    I have just reviewed you schematic.

    1) It seems that the connections of D+ and D- are swapped.

    2) For the RC configuration at the /RESET pin we do recommend 2k pullup and 0.1uF pulldown Cap. D12 Diode can be removed.

    Can you please check this?
