At one of our customers sites, the ISO3082 has burst . Under what circumstances can this part - ISO3082 'burst' ?
If you want to verify the customer's schematic, how can I attach a pdf document in this post ?

Can you explain what you mean by 'burst?'

Hi Jason,
By 'burst', I mean that the IC BURST - there is a HOLE on the top surface of the IC !!!!
I have attached the schematic of this device ISO3082 for yr ready reference

I'm not seeing the schematic - can you try attaching it again? There should be a long URL address inserted into the text of your post when it uploads, but it will just show up as a hyperlink with the name of the file once you submit the post.

I am looking into possible reasons for the IC to pop open. Can you find out what the customer was doing when this happened? I'm wondering whether any of the absolute max ratings were violated. Also, is this happening repeatedly? Or was it just one IC?

There are about 130 units in the field , out of which 4 have 'burst'.
Prima facie the design is OK

Hi Atul,
The IC's may burst when subjected to a heavy transient surge that is above what they are rated for. Typically, external transient suppression is used on the board (near the connector) for this type of protection.
This image showing TVS diodes as protection is from an article written by Thomas Kugelstadt (http://electronicdesign.com/article/power/onchip-transient-protection-74184).
I'm not sure what the application is, but does this sound like what is happening? Any more information you can provide would be helpful.
