TUSB8040A on custom board, no functionality. Any probe points for signs of life?

Other Parts Discussed in Thread: TUSB8040A

I have a custom board with a few TUSB8040A devices on them. The schematic is very close the EVM as I was not able to get an EVM but wanted to play anyways. The power is supplied externally and functioning perfectly. I have 1.131v on the 1v1 rail and 3.357v on the 3.3v rail.

I have LEDs hooked up to the HS and SS Suspend pins as well as the HS/SS indicator port on the chip (A11/A12). I get no light from any of them. It would appear that the chip is sleeping or something is missing.

I am hoping there are some probe points to look for signs of life. Such as, if you don't get X volts out of Pin Y, then it is not receiving a signal there. Or something that I can use to debug why the chip does nothing.

Something that may be of interest is that I am not using the power management features for the downstream ports at all. The downstream ports are powered by an external supply.

Any first clues on where to look?
