TUSB8040 works in USB2.0 mode but does not recognize USB3 device

I copyed the reference design for the TUSB8040 with the exception of replacing the USB3 typeB connector with a USB3.0 micro Type B connector.  When I plug in my design into the usb3.0 microB connector the host computer recignizes the design and "beeps" indicating that the driver is loaded into Windows 7.  If I plug a USB2.0 device like a flash drive/mouse/keyboard the newly added device the computer "beeps", sees and lists the device.  A USB2 flash drive exchanges data to/from the drive/computer with no isses.  When I plug a USB3.0 device into the design - nothing.  No "beep", no indication of activity.  If I partially plug the USB3 device into the connector (emulating a USB2 device connection) the flash drive "beeps" but not in USB3.0 mode. 

I am using a USB3 microB connector and leaving the USB_OTG_ID floating.  My USB3.0 microB connector pinout is: 1 VBUS, 2 DM_UP, 3 DP_UP, 4 OTG_ID (open), 5 GND, 6 CAP_SSTXM, 7 CAP_SSTXP, 8 GND, 9 SSRXM, 10 SSRXP, sheild chassis GND (connected to GND though 1M ohm res/.1uf and .001uf cap in parallel).
