ads7828E/250 10 % failure among 60 piece

HI

       i am using ads 7828e/250 with pca 9306 in proto boards they were fine and even in production of 60 pieces they were fine in testing but when product went out after 1 month or near 6 of them started giving either scaled down values or scaled up values or was not reponding.what should be done regarding this. here is the sch of ads and pca .
