The data sheet for the LMH0307 talks about being able to accept single ended input but only shows how to accept differential input.
What is the proper termination for single-ended input to the LMH0307? The driver is LMH0340.

LMH0340 has integrated 75 Ohm cable driver. Also, LMH0307 has dual 75 ohm cable driver. So i thought perhaps LMH0340 could meet your need so you may not need LMH0307. Please note if you are not using DVB/ASI, you can use both positive and negative side of the LMH0340 since SMPTE is not polarity sensitive(other than DVB/ASI).
I suppose there is a reason you must use both LMH0340 and LMH0307, in this case you can use LMH0340 as a 50 ohm driver(i.e replace 75 Ohm pullup with 50 Ohm) and then AC couple to the input of the LMH0307 with 100 Ohm R on the input. The output swing of the LMH0340 would be less(~600mV) but this should be good enough for the LMH0307.
