TUSB9261 / Firmware Update Failed

I tried to update the firmware of TUSB9261 from version 0.99 to TUSB926x Default Firmware, U1/U2 Disabled (Rev. K)  (ZIP 372 KB )    311 views, 10 Feb 2014 by the tool TUSB926x FlashBurner Utility (Rev. E)  (ZIP 4549 KB )    178 views, 24 Oct 2013.
But I got a message that the FlashBurner Utility wants me to reset the device manually.
After I unplugged / re-plugged USB cable & power cord from the TUSB9261 board, it cannot enumerate anymore.
The device shows Unknown USB Device (Set Address Failed) in device manager.
How to make it back to normal? Thanks.

    Are you using the EVM or your own implementation? Please remove the EEPROM before to connect the TUSB9261 (if you are using the EVM just remove the SPI jumper), then re-connect the TUSB9261and wait until it enumerates as “TUSB926x Boot Loader device” place the SPI jumper and reprogram the device. Then reset the device manually.

     NOTE: please download again the Firmware file from ti.com jus to avoid any file corruption during the download process.
