TUSB8041 Only has USB3.0 working on one port

I'm using the TUSB8041 as part of a design that requires two downstream USB 3.0 ports.  On the boards that we had made for this, we are currently having an issue in which only one of the downstream ports has full USB 3.0 functionality; the other port only appears to support USB 2.0, which is not what we want.  After thoroughly going over the board layout, all the required pins seem to be connected correctly,  and all the layout guidelines listed in "sllsee4b" were followed.  What steps can I take to diagnose the problem here, and to be able to fix it when we redesign the board? 

Attached is a screenshot of the top layer of the board; J3 is the downstream port that is currently able to operate at USB 3.0 speeds, and J2 is the downstream port that is only able to work in USB 2.0.  Above the ports, the TUSB8041 can be seen.
