TUSB7340 configuration EEPROM map

I am an FAE with Arrow Electronics and one of my customers has a question:

The TUSB7340 has the option of adding a configuration EEPROM if the default configuration is not desired.

In looking at the documentation, it appears there are only 2 bits that can be changed (unless I'm reading the wrong document....I refer to SLLA318–June 2012 ), Dynamic D3 and XI_WAKE.

Please verify these are the only 2 options I have and in the documentation, it refers to an offset of 0x15.  Does this mean location 0x15?  The EEPROM is addressed in a x8 mode and the control register in the document refers to a 32 bit word.

Can you provide the EEPROM image of the default configuration as a starting point reference?

For some reason still unknown to us, when we insert a USB3.0 (non-storage) device the unit hangs during boot in the BIOS. We are still working through what the issue could be but there is a thought that we may have to change some of the default settings of the hub.
