TUSB9261, USB recognition is rarely failed

Dear Specialists,

My customer is evaluating TUSB9261 and encountering problem.

Could you please advise.

They made 228 boards on TUSB9261, of which 5 boards is rarely failed  USB recognition.

Please see attached file.

What is the possible reason?

    Hello,
    Please provide the schematic, you can send me a personal message through this forum if you want to keep it private.
    Check the orientation of the TUSB9251 and the physical components for any manufacturing issue.
    Please swap a TUSB9261 from a failing board to a passing board to see if the failure follows the part.
    MAke sure the thermal pad is doing good contact with GND and cold solder is not present.
