TCA9406: request schematic review

Hi,

I'd like to use TI's TCA9406DCUR (I2C Level Translator), and have a problem with debugging my board.

The Symtom is that TCA9406 device is dead, but I don't know why.

Can you review below schematic whether there's any problem?

    Hello TS,

    Do you have decoupling capacitors on Vcci and Vccb?

    Also do you have any startup waveforms?  What are you seeing that is indicating to you that it is a failure?  Have you looking at enable line to see it does see a high?
