Can someone provide a solution to modify the DS30BA101 EVM to use a transformer to couple to 100 ohm twinax cable? The transformer is a requirement for the design - currently a 1:1 with a center tap on the cable side. But we are open to suggestions.

Adding a transformer on the SMA inputs is possible, but it would likely require a seperate diaughter board that sits between the input SMAs on the EVM and the input twin-ax cable itself. From a schematic perspective I would just insert it at the SMA location. You would just need to be careful to ensure the transformer bandwidth is enough for the application.
