Please double check that DP and DM are not swapped.
Can you provide a scope capture of the power-up sequence showing VDD11, VDD33, XI and GRST# ?
The thermal pad must be completely soldered down to the GND plane, are you able to get an x-ray of the thermal pad?

i'm sorry can't provide that, but capture measurement voltage at VDD11, VDD33 and GRSTZ can provide.
