I observe a ripple voltage of 200mV at ~11kHz on pins V+ and V- at 3.3V Vcc and 4 x 100n caps; no load!
Shouldn't that be almost flat at no load or is that within the specification, please?

To generate V+ and V- voltages from the supply, there is a charge pump circuitry internally. Even without load, the charge pump works to keep the output voltage level. You can refer to this post for some technical details and test waveforms.
